Children’s Healthcare Access Program and Prenatal Plus Program Support Services for Obstetrical Practices

 

 

The Colorado Children’s Healthcare Access Program (CCHAP) is a non-profit organization founded in 2006 that provides 14 support services to over 180 private pediatric and family practices throughout Colorado to assist them in providing comprehensive care for children and pregnant women on Medicaid and Child Health Plan Plus (CHP+).  CCHAP, in collaboration with Prenatal Plus (PNP), is now offering a variety of support services for obstetrical practices to make it easier for obstetricians to provide comprehensive prenatal care for pregnant women on Medicaid or CHP+ at no charge to the practice or the patient.

Prenatal Plus is a Medicaid-funded program which provides case management, nutrition, and psychosocial services to Medicaid-eligible pregnant women in Colorado who are assessed to be at high risk for delivering low birth weight infants. These services complement medical prenatal care by addressing the lifestyle, behavioral, and non-medical aspects of a woman's life likely to affect her pregnancy. The Prenatal Plus program has been effective at increasing the number of women who stop smoking, gain an adequate amount of weight, and resolve psychosocial problems, and has decreased the number of infants who are born at low birth weight.

Care Coordination and Socioeconomic Support Services

CCHAP and Prenatal Plus staff provide care coordination and comprehensive socioeconomic support services.  We define care coordination as:  (1) coordinating referrals and services needed by pregnant women; (2) fostering communication and exchange of information about the patient among services and providers; and, (3) encouraging the patient to follow through with all of her care.  Services that CCHAP and Prenatal Plus can provide your practice and patients include: 

  • assistance with financial problems or basic necessities (food, clothing, shelter)
  • assistance with emotional support beyond what the healthcare provider and staff can provide
  • assistance with compliance issues
  • obtaining home health assistance
  • assistance in paying for prescription medications
  • resources for nutrition counseling, smoking cessation, parenting and childbirth education and classes
  • resources for substance abuse, addiction or other distressing or unhealthy habits
  • help in finding specialists for pregnant women on Medicaid as needed
  • help for the women in following through with referrals or appointments (care coordination)
  • assistance with dental and oral health care needs

Eligibility and Enrollment

  • assistance with verifying Medicaid or CHP+ eligibility
  • assistance when a patient has suddenly lost their eligibility/enrollment
  • assistance when a patient needs help with the application/process

Administrative Process, Billing, Coding, Reimbursement

  • assistance with Medicaid or CHP+ administrative processes, coding, billing, reimbursement, or denials

“No-Shows” or Missed Appointments

  • working with the families to determine the causes, and offer possible solutions and resources

Mental Health Assistance 

  • CCHAP has arranged with Medicaid and CHP+ mental health HMOs to respond quickly and effectively to your calls and to find mental health providers comfortable with the care of pregnant women.

Transportation

  • assistance and training in arranging transportation services to urgent and/or routine, scheduled appointments  

Cross-Cultural Communication In-Service Training

  • training and resources for staff or providers on effective methods of communicating with women and families from different cultures
